If you have pets at home, please include their most recent veterinary records with your survey.
Please note that adoption surveys do not reserve a specific puppy. Adoptions will be first come, first served at the event. Please do not arrive more than 30 minutes before the event start time.
Due to the high level of interest, we are unable to answer individual inquiries about specific puppies prior to the event.
