Fred is an approximately 3-year-old male cat who was surrendered on 3/18 due to ongoing medical concerns. He has a history of urinary issues and is currently passing blood in his urine and is dehydrated. These conditions require continuous veterinary care, which cannot be provided at the shelter. Fred urgently needs placement with a rescue by 6 PM tomorrow so he can receive the critical treatment and monitoring he needs.
Medical:
On 3/21 exam Doctor noted: "Unblocked 3/19, passing hematuria. Recheck exam. Quiet, alert, responsive. Moderately dehydrated. Lip licking. Mild dental calculus, no apparent oral masses or foreign bodies. Urinary bladder small and expressible, hematuria.
Body Condition Score (BCS): 7/9
Muscle Condition Score (MCS): 3/3
Assessment:
- Recent UO
- Moderate dehydration
- Severe azotemia
- Progressively elevated BUN, suspected secondary to dehydration and azotemia from UO
Plan
Dx:
- Chem 10: Glu 249 (H), Creat 3.3 (H), BUN >130 (H), TP 9.7 (H), Glob 6.2 (H)"
On 3/19 exam Doctor noted: "Fred was surrendered due to a urethral obstruction. They report him not passing urine or bowels over the past 2 days. When he does eat, he vomits. Not drinking water. Did not take to a vet as previously when he had a scare in the past, it would have been out of their price range to treat and he had gotten better on his own. Quiet, alert, responsive. Euhydrated. Mild dental calculus, hypersalivation, no apparent oral masses or foreign bodies. MM pink and tacky, CRT less than 2 sec. Firm, non-expressible urinary bladder. Nauseous.
Assessment:
- Urethral obstruction
- Azotemic
Plan
Dx performed 3/18 PM:
- Chemistry: Glu 237 (H), Creat 5.9 (H), BUN 84 (H), Na 149 (L), K 3.3 (L)
- AXR: no overt urolithiasis, distended urinary bladder
- Unblocking: area clipped and cleaned. Using a slipper sam urinary catheter, able to pass through urethra without the need for flush. Encountered small area of resistance. Sutured into place and connected to a closed collection system."
On 3/18 triage exam: "Patient came in as an owner surrender and has a history of urinary issues. The owner reported the patient had not urinated for 48 hours and surrendered him due to cost concerns.
Upon presentation, the patient was bright, alert, responsive with a tense abdomen (pain score 4/4), a large, firm bladder, tachypnea (RR 78), and tachycardia. CRT pink/tacky, and temp. 101.0. Dr. will unblock tomorrow morning."
On 3/18 triage staff noted: "Cat was brought into the back office by the owner, who reported that the cat had not urinated or defecated in the past two days. On exam, the cat exhibited an elevated respiratory rate and heart rate. Palpation of abdomen elicited a strong pain response, indicating significant discomfort. Temperature: 101F. Mm pink and slightly dry. Dehydrated (8-10%). Quiet, alert, responsive. Severe pain. Nose clear. Teeth slightly stained yellow with little tartar. Weight 11.4lbs.

Behavior:
On 3/20 staff noted: "When I approached Fred's cage he was laying in the back quietly. I opened the door and reached out to pet him. When I started petting him, he readjusted himself but continued to allow pets. He seemed to relax after getting used to me petting him. He was a very good, shy boy."
